Dr. Hazlett graduated from the University
of Michigan in 1997. He did his AEGD Residency in the Air Force, and then
spent two years at MacDill AFB in Tampa, Florida. He has been in private practice
for three years, owning his own practice in Sarasota, Florida for two years.

CASE
1:
A very elderly,
medically compromised patient, fractured #10, non-restorable due to
decay into biologic width The tooth was extracted by an oral surgeon,
and the patient saw me the next day to deliver the Monodont. Patient
was very happy with the aesthetics, and the fact that she didn't have
to remove it.
CASE
2: Patient
has a combined malocclusion and partial edentulism problem. Due
to the complexity of the case, the patient was referred to a prosthodontist
for an evaluation and treatment plan. The patient was not ready
for what was recommended, i.e. full mouth rehabilitation with fixed
or removable bridges. The patient had since fractured endodontically
treated #7 at the gumline. A flipper was made to replace #7 which
she hated. She begged me to help her.
After thorough informed consent, we decided we'd try a Monodont
to replace #7, keeping the root of #7 to preserve the bone. Impressions
were made and at the next appointment we delivered the bridge, also
doing a porcelain repair on the porcelain fused to metal crown on
#8 with "Sculpt-It" microfill composite. The patient was
very pleased!
